Appointments

You can request an appointment online and then one of our schedulers will contact you to schedule it. Simply click the Request an Appointment button at the top of our website or click here.

Our physician offices are not open on weekends. However, our doctors are on-call at many major hospitals in Greater Cincinnati. If you have an urgent question on a weekend or after-hours, call your physician’s office and listen for the on-call instructions. If this is an emergency, call 911.

You’ll find all the forms you need here.

Please download these forms at the top of our Forms page, print them, and bring them to your first appointment: Patient Information Form, Patient Consent Form, Patient Payment Policy, Patient Review of Symptoms, Patient Medical History, and Medication List.

If you do not have access to a printer, you can complete these forms at your first visit. Please arrive 15-20 minutes early to do so.

We will be happy to see if your insurance is accepted. Simply call the number listed on the page of the Provider or Location you want to visit, and we will be happy to answer your questions.

In most cases you do not need a referral to see a Cincinnati GI physician. When you schedule your appointment, you will be asked for insurance information, which will determine if you need a referral.

Bill Payment

Cincinnati GI offers three ways to pay: by mail, by phone or online. Before you pay online, have your bill handy so that you know what color it is. The color indicates which of our two payment systems you should use. Green bills go to our physicians’ payment system. Yellow ones go to our Anderson Endoscopy Center.

Colonoscopy/Condition Information

Patients are generally advised to get their first colonoscopy at the age of 50, but one may be prescribed sooner for patients whose personal or family history includes colon cancer, gastrointestinal bleeding, or other lower-GI issues. Visit our colonoscopy page for more information.

You can read here about general colonoscopy prep instructions. The literature you received from your pharmacy will have specific preparation instructions. If you still have specific questions after reading it, please call your physician’s office.

Rectal bleeding could indicate a number of conditions. You can read about several of them on our Conditions pages. If you have concerns, please call one of our physicians.

Medical Records

Please contact your physician’s office directly to request a copy of your medical records. There may be a fee associated with your request. You can find a listing of our physicians and their offices here.

We can only provide your records to you or someone you designate on your initial registration form.

In our Patient Portal you can view appointments, test results, and other basic medical information. If you need more detailed information than what is provided in the portal, please contact your physician’s office directly. You can find a listing of our physicians and their offices here.
